Presentation 2: On Their Own Terms: Dialysis Decision-making for Older Adults, presented by Fahad Saeed, MBBS, Assistant Professor of Medicine, Division of Nephrology, Department of Medicine, URMC.

Educational Objectives:

At the conclusion of Presentation 1 participants should be able to:

1.  Understand current data on LVAD therapy and glycemic control.

2.  Understand the limitations of the current data on LVAD therapy and glycemic control.

At the conclusion of Presentation 2 participants should be able to:

1.  Recognize the importance of an engaged dialysis decision-making process.

2.  Learn about the CKD-EDU intervention to improve dialysis decision-making.
