PCMZLs are a diagnostic challenge for hematopathologists, partially due to highly variable morphologic features and a relatively broad differential diagnosis. Many cases also appear to demonstrate plasmacytic differentiation and utilize the IgG4 heavy chain. The diagnostic and prognostic implications of IgG4 usage in PCMZLs have been largely unexplored. We recently compared IgG4+ cases with cases using other heavy chains, and described some new histologic features that can aid in establishing a confident diagnosis.
