Joseph Morelle, Wade Norwood, Chief Executive Officer of Common Ground Health, and Laura Gustin, MPH, Director of the Monroe County Systems Integration Project will present, "Monroe County Integration Project: Building a multi-sector, person-centered system of service delivery."
The social determinants of health are the same as the social determinants of education and of economic prosperity. By March 2024, the Systems Integration Project aims to establish connections between 300 local health, human service and education providers, transforming the way our community works together to help individuals and families seeking support. Project participants are currently designing and implementing community-wide solutions, including an integrated data ecosystem and a multi-sector service delivery model. In this presentation, project leadership will discuss (a) why the Systems Integration Project began, (b) what work is currently underway, and (c) how health professionals might become engaged now, and in the future.
